I have a lame horse with Heel Pain likely navicular any suggestions?

My horse is a 7yr reg. QH. He was used at a University for 3 years. When he because acutely lame on his RF they stalled and buted him for 2 weeks with no response to treatment they called in their vet. The vet checked with hoof testers and he proved sore over his heel. He was a 3/5 on lameness. They performed a diagnostic nerve block on him (PDN) and he went sound then showed lameness on his LF. They didn't do radiographs but concluded he had heel pain and likely navicular disease. They decided trim him and to keep his toe backed up. They gave him Glucosamine IM monthly. In fall he was still lame so they opted not to persue his lameness and turned him out. They dumped him at a sale in DEC. I ended up with him and now I am looking into treatment options for him. I am looking for a good vet and farrier in the Central PA area to work on him. Any suggestions or treatment options will be greatly appreciated. He is sound in a straight line and slightly off at a trot in a small circle.
